Senior Social worker or Team Lead at SAMH YouthReach

YouthReach provides psychosocial support and recovery programmes for youths with emotional, psychological, and psychiatric issues in the community. Our staff work with youths and their families to empower them to achieve their goals and live in a safe, caring, and inclusive community.

Purpose:

You will work with youths, families, and the community to enhance family strengths and resources to provide a better functioning society. By providing case management, counselling, service linkage and coordination in support of youth with mild to moderate mental health concerns in the community. In addition, you will have the opportunity to work with various sources to help the reintegration of clients in need back to the community. The community will also benefit from your knowledge as you conduct outreach programmes and reduce mental illness’ stigma. Responsibilities:

  • Client Management
  • Conduct intake assessment
  • Crisis preparation and safety planning
  • To lead and manage the staff team, providing support and clinical supervision in service delivery
  • Maintaining of progress notes, review of goals and performance at 6 monthly intervals and maintain audit ready files
  • Promote the goal setting and achievement of goals by the client and their respective family members
  • Work closely with families, schools and hospitals and job places to support reintegration of client into community setting
  • Conduct outreach programmes, participating internal and external case conference with community agencies
  • Provide after office hours support to youth, and caregivers as and when the need arises
  • Other duties as assigned by the Agency or Executive Director

Requirements:

  • Degree in Social Work or Psychology
  • At least 5 years of experience in social service sector. Qualification in Social Work & experience in mental health or related work will be advantageous.
  • Patient and passionate about working in social service and with persons of different needs
  • Strong interest to work with youth and their family
  • Ability to work in a team and possess good communication skills
  • Responsible and have good time management skill
  • Ability to adapt to changes and take on extra work scopes
