Senior Case Worker/Case Worker @ SAMH Oasis Day Centre

This position reports to the Deputy Head.

Job Descriptions:

Job Purpose

The role of a caseworker is to provide emotional and practical support to clients and their caregivers in order to help clients achieving their personal goals and improving their lives. Through the application of psychiatric rehabilitation principles, a caseworker instils hope and promote recovery and acceptance for our clients.

A senior caseworker will take a leading role in ensuring the above and be able to conduct peer supervision with the current case management team. He or she is able to identify service gaps, and plan, implement and influence changes within the programme, to better meet clients’ needs, which in turn enhance their recovery processes.

Key Responsibilities

A.  Client Management

  1. To process referrals, conduct assessments and make arrangement for follow-up.
  2. To lead/conduct regular case conference and case discussions within the centre, with other SAMH programmes and external agencies.
  3. To provide individual, group and family counselling & casework in areas such as:
  • Hobbies and interests
  • Employment and work-related matters;
  • Medication management and coping with side effects;
  • Budgeting and financial management;
  • Self-care, activities of daily living (ADL) and household management;
  • Conflict resolution;
  • Interpersonal and family issues;
  1. To work with clients in formulation of their care plans and support them in working towards their personal goals.
  2. To engage and work with clients’ caregivers in order to support clients in achieving their personal goals.
  3. To work with Peer Support Specialist and Activity staff in obtaining holistic overview of clients’ progress in their personal goals.
  4. To conduct home/hospital visits to caregivers and clients where necessary.
  5. To conduct regular psychoeducation workshops and other therapeutic group work for client
  6. To network with community stakeholders and other helping professionals for effective care integration
  7. To evaluate the services provided to clients to determine their effectiveness and prepare recommendations for changes in services delivery as needed

B.  Administrative Support

  1. To write reports, and incident reports, case recording and letters to agencies.
  2. To maintain and compile updated clients’ records and database for statistics submission and monthly reports.
  3. To ensure case files are kept up to date.

C.  Outreach Involvement

  1. To orientate and host visitors to SAMH centres.
  2. To conduct external talks and man info booths to promote awareness of mental health concerns.
  3. To attend invitations for networking sessions and contribute to Ministry-level projects (e.g. focus group discussions)

D.  Other Matters

  1. To seek out continuous learning opportunities for self-development of skills and knowledge (e.g. attend training workshops, participate in community- and Ministry-level projects, involve in work committees within SAMH, etc).
  2. To collaborate with and support other SAMH programmes when required.
  3. Assist in coverage of duties (be it evenings or weekends)
  4. All other official tasks as required by the immediate Supervisor.

Requirements:

  • Bachelor (social work or psychology)
  • Preferred to have experience in mental health field (either through internship, volunteer or work experience)
  • (For senior position) at least 4 years of continuous experience in social service sector
  • Able to speak second language (Mandarin, Malay, Tamil) or dialects (Cantonese, Hokkien, Teochew)
  • (For senior position) Demonstrate leadership, able to provide guidance to SW or CW.
  • Work well in a team
  • Self-driven and Independent
