Case Worker @ Mindset Learning Hub

SAMH Creative MINDSET Hub brings mental wellness to all through meaningful arts experiences. We offer services, including art therapy and occupational therapy, that are integrated with expressive arts. Our integrated model helps individuals relieve stress and improve their quality of life. Many of our activities are open to the public to encourage awareness and reduce stigma for persons with mental illness

Case Management

  • Manage cases and perform case intervention and management to persons with mental health issues or those with multiple disability
  • Conduct referral and pre-intake discussions with stakeholders that include hospitals and other social service agencies
  • Conduct intake assessments for information gathering for assessment of suitability or make relevant referrals
  • Develop and monitor individualized vocational plans and keep proper documentation of session records
  • Provide psychoeducation to enable mental illness management
  • Provide service linkage and referrals for other supports, when required
  • Participate in regular case presentations and discussions within the team, and case conferences with hospitals and other agencies
  • Work with co-workers in formulating individualised learning plan for the development of beneficiaries towards skills enablement and employment
  • Work with community agencies and partners in service delivery
  • Work with employers in the development of jobs and jobs availability
  • Provide pre-vocational support and intervention that include work preparedness, interview skills development and emotional support to beneficiaries during various stages in their vocational rehabilitation
  • Provide training on work skills, social and life skills, when required
  • Present and share centre and SAMH objectives and mission to visitors that include funders, public and representatives from government and social service agencies, schools etc.

Administration and others

  • Complete and maintain proper case notes and session records
  • Process and complete reports required by the centre
  • Work with co-workers in updating and maintaining good database for reporting and tracking purposes
  • Support in centre operations and other ad-hoc duties assigned

Job Requirements

  • Minimum Degree in Psychology, Social Work, Occupational Therapy
  • Experience working with persons with mental health issues and other disability in vocational rehabilitation are advantageous
  • Patient and passionate about working in social service and with persons with different special needs
  • Meticulous and strong in keeping good records
  • Ability to work in a team and possess good communicate skills – able to communicate with employers, co-workers and with management
  • Willing to work in centre (Jurong) and off-site
  • Willing to work on some weekends, when required
  • Ability to adapt to changes
