Case Worker/Social Worker at SAY-IT! YIT@ North

SAMH SAY-IT! (Sports, Arts, Youth Integrated Team) provides assessment and invention for youths and engages youths for ongoing monitoring and support. YIT also works actively with youth agencies, schools and community partners to provide holistic case management for youths and their families.


Responsibilities: 

Client Management

  • Screen referred youth for mental health distress
  • Provide comprehensive, community-based services and support to youth and their families
  • Implement case management service model
  • Conduct intake assessments for referred youths
  • Assist families in crisis preparation and safety planning
  • Formulate individual and family care plans
  • Work with families, schools, hospitals and employers to support client reintegration
  • Strengthen knowledge and ability of families to handle crisis/relapse
  • Provide counselling and support to youths with emotional or psychological difficulties
  • Maintain up-to-date clinical records
  • Manage, supervise and motivate programme participants
  • Conduct support groups and workshops for youths and their families
  • Participate in case conferences with community agencies
  • Collaborate with multidisciplinary teams

Outreach

  • Collaborate with the centre team for outreach initiatives
  • Raise mental health literacy in young people and the general public
  • Network with community partners, hospitals and schools

Administration and Evaluation

  • Perform administrative duties, including case notes, statistics and public inquiries
  • Administer K10 evaluations and document results
  • Assist in revising guidelines, standards, procedures and databases
  • Support Annual Report or statistics collation as needed

Operations

  • Manage day-to-day YIT centre operations
  • Report and follow up on equipment or furniture issues
  • Maintenance of YIT premises

Other Duties

  • Participate in SAMH projects, community outreach programs and events
  • Perform duties assigned by superiors

Requirements:

  • Degree in Psychology, Social Work or related discipline
  • Minimum 2 – 3 years of relevant work experience
  • Experience working in Social Service Agencies, especially youth work will have an added advantage
  • Motivated and passionate to serve youths in mental health sector
  • Good interpersonal and communication skills
  • Organised and timeliness in keeping up with documentation
  • Insightful and reflective
  • Willing to learn, adapt and improve oneself
  • Able to work independently as well as in a team

Skills and Competencies 

  • Must be proficient in MS Word, Excel and Powerpoint
  • Experience with facilitation and case management counselling will be an added advantage
  • Experience with and having skills in the arts (visual arts, music, drama, dance and/or sports will be an added advantage
